In order to nominate a baby for a replacement Nanny BM-02 mattress pad, the baby needs to be enrolled onto the CONI programme by completing the online Enrolment Form.

Important: Please ensure that you have consent from the family to share this information with the Lullaby Trust. We will only use their contact details to send the equipment. If this is not possible/advisable or no consent to share contact details, then the monitor will be sent to the Coordinator to be provided for the specific nominated baby.
The Nanny mattress pad will be sent directly to the parent(s). The Nanny mattress pad and control unit should be returned to the Coordinator at the end of the programme. The CONI Coordinator is responsible for the safe disposal of the Nanny mattress pad locally.

The Nanny BM-02 Mattress monitor pad will be sent by 2nd Class Royal Mail. In exceptional circumstances express delivery can be arranged with the CONI Team. Email coni@lullabytrust.org.uk with the reason for the request.
