Snuza Nomination Form

Before you begin the nomination process, please ensure you are familiar with your responsibilities and everything the family will need to know about receiving a Snuza monitor.

Snuza Nomination Guidelines

  • In order to nominate a baby for a SNUZA monitor, the baby needs to be enrolled onto the CONI programme by completing the online Enrolment Form. Also, before you complete and submit the nomination form please discuss this arrangement with your manager who should seek local legal/governance advice.

  • Coordinator details

    If monitor is to be dispatched to coordinator not parent
  • If you don't remember your ID please contact the CONI team at [email protected]

  • Parent details

    Please ensure that you have consent from the family to share this information so please ensure that you complete the section at the end of the form. The SNUZA monitor will be sent directly to the parent(s) as the monitor is their property. Please advise them of this. If this is not possible/advisable or no consent to share contact details, then the monitor will be sent to the coordinator to be provided for the specific baby.

  • The SNUZA monitor will become the property of the parent(s). Please do not retrieve the monitor at the end of the CONI Programme.
